The service to be provided to you the “Recipient” will be performed by Robin Futoran, D.C., FACO and/or Associate, and referred here as “Healing Practitioner” (HP). This is not a medical treatment, chiropractic care or any other form of healthcare. It is a healing technique that utilizes concepts combining Physical, Medical and Metaphysical knowledge and science with the intention of stimulating positive health oriented changes in the body. The process is designed to open channels of energy from highly focused higher consciousness and thought energy, tapping into healing frequencies of universal consciousness and increasing your physical and spiritual sensitivity to the natural healing processes of the body. BCH is additionally a system of teaching you the “Recipient” a method of working within your own abilities to stimulate healing such changes in your body.
The theory is based on the premise that “thoughts change biochemistry.” You will not be examined nor diagnosed or treated regarding any physical or emotional condition. You will be guided within the parameters of BCH, with the intention of stimulating positive healing changes of body, mind and spirit.
You are requested to provide details regarding your condition, laboratory findings as BCH is in part a visualization process whereby an understanding of what the condition looks like will assist in visualizing the dis-ease and intended restorative changes. The results of your sessions may be archived without using your name for study and research purposes.
By signing this document or electronic agreement, you understand that there are no promises or guarantees regarding results or outcomes from these BCH healing sessions.
Confidentiality: All information received from a you is strictly confidential and will never be released in any form without written authorization from you. Hard records will retained and may be part of an ongoing study, though you will remain anonymous.
Confidentiality might be breached if you are an imminent risk to yourself or make threats of violence against another person allowing the appropriate authorities in taking protective action for you or others. If such a situation takes place during our work, every effort will be made to first discuss the issue with you the Recipient prior to taking any action.
As you are aware, it is impossible to fully protect confidential information that is transmitted electronically even with email and information stored on computers that are connected to the internet or over cell phones. I do maintain firewalls and have the standard securities that come with personal computers and can not be responsible for information that might be hacked or intercepted through online connections without my knowledge. Beware any time you send email or talk on a cell phone there is a possibility that this information could be intercepted. Any information that might be too sensitive for publication should be directly discussed over a land line telephone or in writing through the mail.
AGREEMENT:
This document and attachments constitute an agreement between you the Recipient and myself as the HP, administrator of the HEALING MANKIND PROJECT or other HPs. You are required to read this agreement carefully, and raise any questions or concerns you might have prior your physically or electronically signing, initialing and returning by email, fax or mail.
Release and Indemnificaiton: Recipient, Participant, Healing Practitioner, on behalf of himself and his respective successors, executors and assigns, fully and forever release and agrees to indemnify, defend and hold harmless Robin L. Futoran, The Healing Mankind Project, Bio-Cognitive Healing Technique, The Reconnectionâ and their officers, members, managers, representatives, affiliates, and each of their successors, assigns, estates, beneficiaries, legal representatives, agents, employees, attorneys, insurance carries, hosts, venues, volunteers and independent contractors, in perpetuity, of and from any and all manner of claims, liabilities, obligations, actions and causes of action (including attorneys’ fees) (Claims) which Recipient, Participant, or Practitioner may have arising out of or in connection with the work, workshops, techniques, information and services. “Services, Workshops, Seminars,” shall be defined as any past, present, and/or future lectures, events, book signings, workshops, seminars, healing sessions, lessons by Robin L. Futoran or representative of The Healing Mankind Project and Bio-Cognitive Healing Technique, in any medium now known or hereafter devised, including but not limited to radio, live, television, and the internet, or use or receipt in any manner and in any form of the information, lessons or services.
Arbitration and Jurisdition: Any controversy or claim involving the construction, enforcement or application of any of the terms, provisions or conditions of this Agreement shall be settled by binding arbitration in Los Angeles, California or such other location designated by The Healing Mankind Project before the American Arbitration Association (“AAA”) pursuant to its Commercial Dispute Resolution Rules or any comparable rules then in effect. Judgment upon the award rendered by the arbitrator(s) may be entered by any court of competent jurisdiction. The costs of arbitration shall be borne equally by the parties with each party responsible for his or its own attorneys’ fees. The arbitrator(s) shall have the authority to determine all issues of arbitrability. The arbitrator(s) may not award punitive or exemplary damages. You hereby waive all rights to such damages and to trial by jury. The decision of the arbitrator(s) shall be kept confidential by the arbitrator(s), the AAA and the parties.
The service to be provided by the designated HP(s) including BCH is described as “promoting physical well-being through guided meditation, affirmation, intention, thought, and prayer via highly focused visualization and intention,” in an effort to stimulate the body’s natural healing capabilities. Provided services are not medical, chiropractic, diagnostic or any other form medical or healthcare and that no claims promises or guarantees are made. This is not a substitute for medical or psychological diagnosis and treatment. I understand that I should not discontinue any medical treatment unless advised to do so by my licensed healthcare provider. I further state that I am seeking these services of my own free will and without guarantees.
Once this agreement has been electronically accepted, payment is required at the beginning of an appointment paid in advance. A Paypal link will available to accept payment by credit card.

You are required to give 24 hours notice if you need to cancel or change the time of an appointment. Otherwise, you will be charged for the session in full. Dr. Futoran will make every effort to reschedule sessions which are cancelled in a timely manner.
